Life Insurance

Three life insurance plans are currently offered by the AgCenter.

  1. Prudential (through the Office of Group Benefits)

  2. LSU Supplemental Life (UnitedHealthcare)

 



PRUDENTIAL
(Group Benefits)
 - A group life insurance plan is offered through the Group Benefits Program. Half of the premium is paid by the state for the employee's coverage. The employee must pay the entire premium for dependent coverage. The employee may select from two options with a maximum coverage of $50,000.

BASIC PLUS SUPPLEMENTAL PLAN

Employee Coverage

To calculate the face amount, multiply annual salary times 1 1/2 and round up.  If the result is less than $21,000, add $1,000.  (Maximum Coverage is $50,000)

LSU SYSTEM TERM LIFE INSURANCE
(Administered by UnitedHealthcare) - This plan provides an option for group-term life coverage for eligible employees. It provides a death benefit equal to the coverage amount in effect at the time of death and payable to the named beneficiary. Coverage amounts available range from $10,000 to $1,000,000. Premiums are age-rated. Also available is accidental death and dismemberment coverage.
Dependent life insurance can be purchased for the employee's spouse and children.  Coverage for children may be added anytime without proving Evidence of Insurability.

Eligibility for Enrollment

Employees appointed for more than 120 days and 75% of full-time or greater are eligible for enrollment.

Effective Date of Coverage

  • New employees have 30 days to either enroll for or waive coverage under this plan. Coverage is effective on the first day of the month after completion of one full calendar month of employment.
  • Employees who do not enroll for coverage within the first 30 days but who later wish to pick up coverage must complete the Evidence of Insurability form, send the entire document to the HRM office for completion, and wait to be approved for coverage.

Within 30 days of new hire, the plan guarantees issue of coverage up to 5 times your annual salary (in $10,000 increments) or $500,000, whichever is less. Premiums will adjust every January 1, according to age. Premium Calculation sheet is linked below. Once enrolled, the maximum coverage is $1,000,000. Any amount applied for or purchased over $500,000 will require evidence of good health/insurability. The form is also linked below. If elected, your AD&D policy will be equal to the term life insurance amount.

Spouse coverage is available up to 50% of the amount of employee coverage, but only up to $100,000 when enrolled within the first 30 days of new hire or within 30 days of a family qualifying event. AD&D coverage, equal to the approved life coverage amount, is available.

Children may also be covered with no medical information required. Amounts are $5,000, $10,000, $15,000 and $20,000 for each eligible dependent child (14 days - 26 years). AD&D coverage also available to match life amount, not to exceed 50% of employee's benefit amount. It is the employee's responsibility to notify the Human Resource office to stop deductions once a child is no longer eligible.

  *Accelerated Death Benefit (The life Insurance Benefit amount will be reduced by the amount paid under the Accelerated Death Benefit.)

EXTENSION BENEVOLENT AND PROTECTIVE INSURANCE
This is optional life insurance coverage offered to all Louisiana Cooperative Extension Service appointed employees.  In addition to life insurance, it also provides for Accidental Death an Dismemberment (AD&D) coverage.

  • The effective date of coverage begins on the first day of the month following the date of employment.
  • The face amount of coverage reduces at age 65 and again at age 70 (or upon retirement at any age).
  • The AD&D portion of the coverage cancels at age 70 or upon retirement.
