The following section provides the appropriate contacts and/or forms required when a retiree has an address change or a change in family status which affects insurance and/or retirement benefits.  It also includes a section on returning to work after retirement.  Click on a link to go directly to that section:

Address Changes
  • Insurance - In order to change your address on your records with LSU and all insurance programs, you must submit a letter requesting the address change.  The letter must include your name, social security number, new address and must include your signature.  Mail the letter to:

Beneficiary Changes

In the event of marriage, divorce, or death of a spouse/dependent, you may want to consider changing your beneficiary on any life insurance policies.  Please contact Kathy Loyd in the AgCenter HRM Office at (225) 578-8229 for the appropriate forms.

Benefit Changes as a Result of a Change in Family Status

  • Death of an LSU Retiree - Surviving Spouse/Dependent Health Coverage - A surviving spouse or dependent of a deceased LSU retiree who had health insurance coverage may be eligible to continue this coverage. Contact Kathy Loyd in the AgCenter HRM Office at (225) 578-8229 to determine eligibility. Notify Ms. Loyd within 30 days of the death of the retiree. Surviving spouses and/or dependents will have 60 days to decide if they wish to continue coverage.
    • Kathy Loyd will send you the appropriate form to file for life insurance, if covered. A certified death certificate will be required, so one should be requested as soon as possible.
    • Ask about changing your beneficiary on life insurance policies.
    • Notify the appropriate retirement system immediately:
      • CSRS (1-888-767-6738)
      • LASERS (1-800-256-3000 or 225-922-0600)
      • ORP
        • AIG-VALIC (225-931-9090)
        • ING-AETNA (225-766-8711)
        • TIAA-CREF (1-800-842-2006)
      • Social Security (1-800-772-1213)
      • TRSL (1-877-275-8775 or 225-925-6446)

  • Death of a Spouse or Dependent -  Notify Kathy Loyd in the AgCenter HRM Office at (225) 578-8229 as soon as possible. Notifying this office within 30 days will allow the reduced insurance premium, if applicable, to be effective at the earliest possible date.
    • Complete the following forms (if enrolled) and return to the AgCenter HRM Office, P.O. Box 25203, Baton Rouge, LA, 70803-5203:
    • GB-01 - Health Plans and/or Group Benefits Life (Prudential)
    • Dental Change Form
    • Vision Change Form
    • Kathy Loyd will also send you the appropriate form to file for life insurance, if covered. A certified death certificate will be required, so one should be requested as soon as possible.
    • Ask about changing your beneficiary on life insurance policies.
    • Notify the appropriate retirement system immediately:

      • ORP

  • Dependent No Longer Eligible - A dependent child will no longer be eligible for coverage when the child reaches a specific age or when the child marries at any age. You should notify the AgCenter HRM Office (225-578-8229) within thirty days of your dependent becoming ineligible to continue benefit coverages.  The specific age that coverage for a child will end is determined by whether or not that child in enrolled as a full-time student as follows:
    • coverage ends at age 21 if the child is not a full-time student, or
    • coverage ends at age 24 if the child is a full-time student.
      • if graduation occurs in the spring, coverage will continue through the end of August.
      • if graduation occurs in December, coverage will continue through the end of December.
    • Timely notification will allow your child to be offered COBRA continuation benefits as soon as eligible.
    • Coverage may be canceled at any time by completing the following forms, if enrolled, and returning to the AgCenter HRM Office, P.O. Box 25203, Baton Rouge, LA, 70803-5203::

  • Divorce - As soon as your divorce is final, you should notify the AgCenter HRM Office within thirty days to remove your ex-spouse from your benefit plans.  Timely notification will allow your ex-spouse to be offered COBRA continuation benefits as soon as eligible.  The following forms, if enrolled, must be completed and returned within thirty days to the AgCenter HRM Office, P.O. Box 25203, Baton Rouge, LA, 70808-5203:

  • Marriage - If you wish to add your spouse to your insurance policies, you must do so within thirty days from your date of marriage in order for coverage to be effective retroactively to the date of marriage. If you wait more than thirty days, your spouse will be treated as a late applicant and must prove evidence of insurability.  The following forms, if enrolled, must be completed and returned within thirty days to the AgCenter HRM Office, P.O. Box 25203, Baton Rouge, LA, 70808-5203:

Returning to Work (Re-employed Retiree)

There are many different factors, including the retirement system from which an individual is retired, which determine how and when a retiree may be re-employed.  You should  contact Kathy Loyd in the AgCenter Human Resource Management Office at (225) 578-8229 to discuss your particular situation.
