2009 Incentive - CSRS CSRS - Civil Service Retirement System In addition to the Retirement Incentive Program Application Form, you will need to complete and submit the following to the HRM Office no later than the close of business September 30, 2009. Service Retirement: Application for Immediate Retirement (SF 2801) - Sections A, B and I TRSL-SB: Application for Service Retirement (Form 11A) - Sections 1 and 3 Note - Complete the TRSL-SB only if eligible for and electing the TRSL-SB at time of CSRS retirement. __________________________________________________ Additional Documents Necessary to Complete the Entire Retirement Application Package : Please Note: Entire applications must be completed for retirement purposes.  Sections A, B and I of SF 2801 and Sections 1 and 3  of the TRSL-SB are sufficient only to meet the requirements to participate in the Incentive Plan, if submitted before September 30, 2009.
