After you have submitted the Request for Employment Packet form to the HRM Office and received the email stating which forms are required, print out the required forms for each folder and provide to your new employee for completion.

- Form I-9 - Employment Eligibility Verification
- Bank Deposit Authorization Form (Electronic Direct Deposit)
- Drug Policy Statement (AgCenter PS-18)
- Bank of America Corporate Card - (For Academic & Non-classified Employees Only)
- Car Insurance Agreement - (For LCES Academic Employees Only)
- Name Tag Information - (Complete and sign if you will serve in a capacity to represent the AgCenter away from the office.)
- Selective Service Certification - For all male U.S. citizens and male resident aliens living in the U.S. who are ages 18 through 25.
