After you have submitted the Request for Employment Packet form to the HRM office and received the email stating the benefits for which your new employee is eligible, do the following if applicable:

Print out the documents under
Forms, Guides and Instructions
and give to your new employee.

Have the employee view the health plan information on this site directly.

If the employee does not have access to this site, request a hard copy packet from the HRM Office to provide to your new employee.

Appointment is equal to or greater than 121 days

Appointment is equal to or greater than 75% of full-time
