Documentation Required For Sampling

The Chain of Custody form is required to accompany regulatory samples from EPA- and LDEQ-funded projects. All other samples must be accompanied by a Sample Analysis Request Form. Please review the Water Sample Management and Collection Guide in the "Additional Information" link for necessary sampling and shipping guidance.
