Creating a Portrait of you: The Record Book
Materials Needed:
- Pen or pencil
- Project Book(s)
- Binder
- 4-H Portfolio (7th-12th graders)
- Pictures, supporting material
- The willingness to showcase what you have learned
Instructions:
- Read the guidelines that explain each section.
- Punch holes, if necessary, and bind your 4-H Portfolio in your binder.
- Be sure to turn the book in to your 4-H leader by the deadline to be sent to the 4-H Office.
- Use divider pages for all sections.
- Write name and club on outside of binder.
- Showcase only three years worth of work.
- Do not use protective sheet covers. Optional for the photo and supporting material section.
Record book is put into the following order:
Section 1 Personal Data
*Photo of yourself
*Name
*Age, Grade, and Date of Birth
*Address
*Parents/Guardians Names
*List Projects (project books) enrolled in
*Signatures (you, a parent/guardian, local club leader)
*Answer the four questions on 4-H and You!
*Remember to print neatly. (7th -12th grade may type this section)
Section 2 4-H Leadership
*Fill out for each year you have been in 4-H. Put most current year information in first.
*List any offices or committees you have held.
*List any time you taught, coordinated, organized or guided others.
*Be descriptive about activities and use action words.
*Remember to print neatly. (7th -12th grade may type this section)
Example: Club Demonstration- Taught 34 club members how to do a method demonstration.
Served as 4-H Week Chairman for school club. Prepared daily activities to celebrate National 4-H Week.
Section 3 Citizenship/Community Service
*List community service activities you participated in. Include hours worked/ type of service and number of items donated or collected.
* If you did nothing, write none, and try to be more active next year.
Section 4 Awards & Recognition
*List any awards you received in 4-H contests this year. Start from Achievement Day.
*DO NOT PUT RIBBONS/CERTIFICATES IN BOOK!! Please take a picture of awards or certificates and place in supporting material section.
Section 5 Non-4-H Experiences-Leadership
*Place non 4-H work here such as Dance Team Captain, FFA President, President of the Louisiana Jr Cattleman Association.
*Fill out for each year you have been in 4-H. Put most current year information in first.
*List any offices or committees you have held.
*List any time you taught, coordinated, organized or guided others.
*Be descriptive about activities and use action words.
*Remember to print neatly. (7th -12th grade may type this section)
Non-4-H Awards & Recognition
*List any awards you received in this year. Example: Perfect Attendance at School, Outstanding Salesman Girl Scouts.
*DO NOT PUT RIBBONS/CERTIFICATES IN BOOK!! Please take a picture of awards or certificates and place in supporting material section.
Section 6 My 4-H Story
*Write a 4-H story about your work in your 4-H project(s) and other 4-H activities you were involved in this year.
*Come up with a catchy story or slogan to begin your story.
*Tell us why you joined 4-H and why you selected your projects.
*Describe some of the interesting things you did and what you’d like to do in 4-H in the future.
*Limit your story to no more than 6 pages.
*Remember to print neatly. (7th -12th grade may type this section)
Section 7 4-H Photos and Supporting Materials
*If you do not have any pictures, take some now & have them developed.
*Do not layer pictures or supporting material.
*Pictures should be used to show your project accomplishments, leadership activities, and recognition.
*Attach with rubber cement.
*All pictures must have a caption or description of what the pictures show.
*Do not write on the pictures.
*Don’t put any pictures on the back of a page. Add additional pages if needed. No more than 10 pages total.
*You may include any materials containing your name in print such as 4-H News Clippings about yourself or letters of recognition or thanks you have received concerning your 4-H work.
*May use protective sheet covers on this section.
*Select only those that you feel will help support your 4-H project accomplishments.
Section 8 4-H Project Focus
*Place your completed project book(s) behind this section. Complete a separate Section 8 for each project submitted with your portfolio.
*Remember to print neatly. (7th -12th grade may type this section)
* Here is where you showcase what you have learned in that project. Tell about something interesting you learned or how long it took you to sew on a button. Did you interview a veterinarian about taking care of your pet? Did you spend a day with a photographer and what did you learn? Did you show your family how to use the food guide pyramid? What did you like and dislike about the project?
Project Books:
*Should be placed behind their own section 8.
*Livestock projects members should fill out a livestock record form. Livestock record sheets should be placed before the project book.
*Should be in your own handwriting.
*Completed the recommend number of activities in each project.
*Place pictures of you doing the activities into your project book. Example: If you are in the Six Easy Bites book and are baking cookies, take a picture and place in the book.
Enjoy your record books. This is your time to shine and demonstrated to others what you have been learning.
